Referral privacy

How we use your referral information

We want you to know what happens to the information you share when you ask Advocacy Bridge Tennessee for help.

What the form asks for

Our initial referral form asks for your name, contact information, county, the kind of support you need, and any upcoming deadline you want us to know about. A professional making a referral should have the person's or parent or guardian's permission before sharing their information.

What we do with it

We use your answers to contact you, review whether we can help, and keep track of your request. Microsoft Forms receives the form, and we use Microsoft 365 tools to manage referrals. People working on a request may use the information needed to handle it.

What to leave out

Please do not include IEPs, evaluations, medical records, financial information, or other sensitive documents in the initial form. If we need more information, we will contact you about what to share and how.

Before services begin

Submitting the form does not start services. There is no charge for the initial review. We currently accept private-pay clients only, and we discuss fees before any services begin.

Questions or corrections

For questions about your referral information, including how long we keep a request, or to correct information you sent, email referrals@advocacybridgetn.org or call (731) 248-8469.
